What Are Custom Forms?
Custom forms in DigitalTCO are consent forms that can be accessed and managed within the platform. These forms are designed to streamline patient communication by allowing practitioners to create, send, and store consent documents relevant to a patient’s specific treatment plan.
Where to Find Custom Forms
- On the left-hand side menu, navigate to Communication.
- Click the drop-down menu, then select Custom Forms.
- A list of available consent forms will be displayed.
- These forms can be sent to specific patients based on their treatment needs.
How to Add and Edit Custom Forms
If you need to create or modify a custom form, follow these steps:
- Navigate to Settings:
- On the left-hand side menu, click Settings.
Open the drop-down menu and select Templates.
Access the Custom Forms Section:
On the top banner, click Custom Forms.
Create or Edit a Form:
- On the right-hand side, click Add Form.
Enter the required information and make any necessary edits.
Save Your Custom Form:
- Always press Save to ensure your changes are recorded.
